What is Hospice Care?

Hospice Care is a compassionate philosophy designed to enhance the quality of life in the final months by providing comfort and dignity. Our team of dedicated professionals, volunteers, and family members work collaboratively to ensure that no one faces this journey alone. What is Palliative Care and How Do You Find Support?

Palliative care addresses the whole person and their family, not just the illness. It is available to individuals of any age, in any setting, delivered by healthcare providers, family, and caregivers. Palliative care supports people who are living with life-limiting illnesses. For personalized home care assistance, connect with Home and Community Care Support Services (HCCSS) or call 1-800-811-5146. If you have any other questions our nurses will help guide you through the next steps.

Submitting a Referral:

Your HCCSS Care Coordinator can determine eligibility and complete a hospice referral. If considering relocation, the local HCCSS must send a referral to HCCSS SW.

What is Residential Hospice Care?

Experience specialized care in a homely environment during the last months of life. Our hospice care philosophy goes beyond physical care, encompassing social support and person-centered care. Join us in creating a comforting atmosphere for individuals and supporting their loved ones through this journey.

Is There a Waitlist?

JHH, like many other hospices, doesn't use traditional waitlists for admission timing. While we maintain a referral list, not everyone referred is immediately ready for hospice care. Eligibility doesn't equate to suitability. We regularly update our referral list and prioritize admissions based on various factors to ensure those most in need receive care when beds become available.

What Other Services Do We Offer?

Explore grief and bereavement services, community education sessions and workshops, caregiver support, and assistance with end-of-life decisions. Hospice is a comprehensive experience beyond the last weeks of life; it is about supporting the entire palliative journey.

Is There a Cost Involved?

Hospice services are free when admitted to Jessica’s House Hospice. Third-party services may incur expenses, such as medical transport and medication not covered by the Ontario Drug Benefit or third party insurance.

How to Qualify for Hospice Residence Care?

Admission criteria typically involves the diagnosis of a life-limiting disease, a DNR-C order, and a preference for comfort care over curative care.
